Three people have died following a serious multi-vehicle crash in Lakeshore, near Windsor, Ontario, on Wednesday morning, October 28, 2026, according to Ontario Provincial Police (OPP). Investigators say the collision began when a vehicle travelled in the wrong direction on County Road 22 and struck other vehicles.

Emergency services were called to the area at approximately 6:07 a.m. after reports of a major collision. Police launched an investigation into the circumstances surrounding the crash, which involved six vehicles and left several people with life-threatening injuries.

Wrong-Way Driving Led to Fatal Collision

According to Essex County OPP, the preliminary investigation indicates that a vehicle was travelling against the normal direction of traffic when it struck another vehicle. Police said the initial impact caused minor damage and did not result in any reported injuries.

However, the vehicle travelling in the wrong direction continued and then collided head-on with a second vehicle. The force of the second collision caused life-threatening injuries to both drivers.

Police confirmed that both drivers were pronounced dead at the scene.

The crash also seriously injured passengers travelling in the two vehicles involved in the head-on collision. One passenger from each vehicle was transported by ambulance to hospital for emergency treatment.

One of the injured passengers later died at the hospital, bringing the confirmed death toll to three. The other passenger was airlifted to London Health Sciences Centre for further medical care.

Authorities have not provided further details about the identities of the victims or the condition of the surviving passenger in the information released so far.

Six Vehicles Involved in Lakeshore Crash

Investigators confirmed that six vehicles were involved in the incident on County Road 22. While the initial collision and subsequent head-on impact resulted in fatal consequences, the other four vehicles sustained light damage.

Police said the occupants of those four vehicles did not report any injuries.

The West Region Traffic Incident Management and Enforcement Team, known as the TIME Team, attended the scene to assist Essex County OPP with the investigation.

The crash prompted a temporary road closure as emergency responders and investigators worked at the location. County Road 22 was closed between Manning Road and West Pike Road for several hours.

Motorists were asked to avoid the affected stretch of roadway while police carried out their work and crews managed the scene.

Investigation Continues

The fatal collision has raised concerns about the dangers of wrong-way driving, particularly on busy roads where drivers may have little time to react to an approaching vehicle.

Police have identified wrong-way travel as the starting point of the crash, but further investigative work may be needed to establish the full sequence of events and the circumstances that led to the vehicle entering the opposite lane.

As the investigation continues, authorities may release additional information about the victims, the vehicles involved and other relevant findings.

The incident occurred in Lakeshore, a municipality in Essex County, east of Windsor, Ontario. The deaths serve as a reminder of the serious consequences that can result from dangerous driving and unexpected hazards on the road.

This report is based on information published by CBC News and statements attributed to Ontario Provincial Police. Further details remain subject to official confirmation.
